It looks like its doing what it is supposed to. When you install new (multiple new anyway) flash, it may appear as corrupted or as separated partitions.
As for your TFTP, it looks like it started just fine in terms of asking you the questions and erasing the current flash. So the question is where is that TFTP server in relation to your router? Is it on a locally connected subnet? If not, do you have a route or ip default-gateway to get there? With brand new flash, was it properly initialized as 32M when you did "show flash"?
