I had a similar problem and found that I had forgotten to put a default gateway in my scope. In that case you would see the offer, the phone would accept it, but then it wouldn't complete the accept, because the acceptance wouldn't make it to the server without a default gateway.
Another stupid question. Your phones are requesting dhcp, right. They weren't configured for static ips, were they?
