I think you're talking about "Operation Mincemeat", the subject of a quite good recent 
movie.  The body in question was Welsh, not an actual officer but a "legend" was created 
out of whole cloth for an imaginary officer.  See Wikipedia for the whole story,  which includes a 
picture of his grave marker that shows both names.
