For general documentation I generally stick with the preference of
explaining it upon the first instance, then leaving it alone for the rest.

If the documentation is for training purposes or is task instructions it's
sometimes better to bold every instance - sometimes this helps those
scanning the docs to complete tasks (since, presumably they're moving their
eyes between the page and the screen for the duration of the task).
