I have heard tha tthe best thing to do in those situations, if one has 
the presence of mind, is to make eye contact with one person and tell 
them to help you.  Don't ask or beg.  You make it an order.

Supposedly, the person singled out loses the anonymity of the crowd and 
also the idea that"someone else will do it".  It puts them on the spot 
all of a sudden they look like an ass if they don't do something.  Once 
that person acts, other usually follow.

It doesn't always work, but it is worth a try.
