The Hawaii plane lost it's upper casing for something like 10 rows.  It 
was caused by metal stress and turned out that it was a known and 
preventable problem that the airline had failed to implement.

As far as some nutcase charging the pilot/co-pilot and them missing, 
you're talking about a relatively small hole.  Depressurising a cabin 
does NOT cause a plane to crash.  Not even loss of a primary flight 
control system guarantees flight failure.
