Our basic setup has developers using studio and some source control
software (sometimes VSS, sometimes PVCS, sometimes another, depending on
the project) to develop the app on the development site, which often
involves less robust hardware than staging or production boxes. The lead
developer holds the controls to determine when to publish code up to
staging. The staging servers are a clone of the hardware in production,
including load balancing. Once on staging, the code goes through a series
of QA/Load/Acceptance testing. Once this is complete and acceptable,
another set of scripts copies the code up to the production servers.
