We removed CFMX (completely) from a server - rebooted - then installed CF5 and rebooted. Have also installed all patches for CF5. Have followed all the suggestions from the MM FAQ regarding this (Getting a Name and Password Challenge Response When Requesting a ColdFusion Page).
Still getting it.
