Again, AFAIK, it effects all platforms

And as for "certain conditions", I think that lands under a "when it
feels like it" scenario

I've never seen a definite situation where it always occurs, if you send
5 emails, sometimes you'll get all 5, sometimes you'll get 2 or 3

I'm not sure how it works for Linux, but you can get replacements for
CFMAIL, several companies sell simple things to do it - maybe investing
in one will solve your issues
