Perhaps the livedocs should just say case-sensitive? Maybe that's what they meant. They're probably just making a point to show that most tags are case-insensitive in CF, but because custom tags make a call to the file system, the tag name and the file name must be the same case. It's probably just easier to explain it as "they must be lowercase" than to go into the explanation.
