We use StarTeam at the office and I love it.  Full workflow management from
change Request to deploy, defect tracking, threaded discussion engine
("topics" can be linked to anything - a file, a change request, a defect
report, a folder, etc) and so forth.

It is on the pricey side however requiring both a server and client license
(although there's also a nice web client that's useable for most tasks and
especially valuable to the "business" for writing up defects and requests
without having to pay for a client-license for each).
