I had to pull big pups out before too.  I never let the mom’s eat all the 
placentas either. I just let her have a few.  :-) -- em

in this case this "midwife" had to pretty much pull the 1st pup out...it
was breached and its big head didn't want to come out and when it finally
did, it had been without sufficient oxygen for some time and I couldn't
find anything to suction the fluid from its nose and mouth so I used my
own mouth!!! lol. Also, I always cut the cord so momma don't bite too
close or pull too hard, help break the bag and clear the airway, make sure
all the placenta is delivered, ect.....
