That is an old wives tale that dogs can have a double or a delayed 
insemination during a single cycle.  Most pregnancies take place during
12-16 day of a bitches cycle when mated,  and pregnancy can only happen once
 only one sperm cell can make it and all the other sperm cells die off.

The runts are from poor genetics and BYBs & PMs breeding there already
smallest chis hoping to get even more runts. However, if a healthy line has
been proven of small chis like 3-4 pounders which is what most show chis are
 then you can keep a good line going. I am not saying that even in a healthy
line there won't be a couple of defects, because that happens in any breed
of dog, it's just the way genetics work, eventually weak genes will show
through a line in some generations.

Some of the BYB erroneously called teacups are runts. BYB and PMs will let
the male and female mate several times and the due date is from the first
breeding that took. So if allowed to breed for 5-7 days a pup can be 5-7
days early and a runt. Since they are premature the skull is not full formed
along with other things, lungs etc just like a premie human.

That's the very 1st thing the vet checked for when I took my Chi pup in for
a check-up. I told her, "Nope, she doesnt have one" as soon as I saw her
feeling her head lol is there any rhyme or reason as to which pups have that
soft spot? Or is it just random?

Molera is a soft spot in some Chihuahuas skulls and people pups too. It is
where the skull has not completly closed so only skin and membrane sheilds
the brain. Your vet would know if your pup had one and should have warned
you.
