The best implementation for me would be the simplest as well.  Just have an  
option to
turn off the header and footer and print the page like it is on the screen.

In Firefox, that is no margins, shrink to fit page width.  That would fill  
the
printable area with what is on the screen, print pdf's, mailing labels, and  
any print
ready page correctly.

My only concern on this is that some printers may need margins.  My HP  
Laser Jet does
not have a problem with a file like this  
http://allen-edwards.com/test.html.  I use
"margins = 0" and "shrink to fit". This setting also prints web pages just  
like they
look, which I think is the real goal behind this feature request.

In my mind, it is actually better to just have a simple turn off headers  
and footers
rather than a full UI for settings.
