Even standard app-global commands like Cmd-Q and Cmd-W don't work when a  
plugin has focus. This is directly contrary to
Mac UI guidelines (and common sense.) This annoys me many times a day. The  
menu bar takes precedence over the focused UI
component when handling command keys.

That's a cross-platform fallacy. A typical user uses different apps on one  
OS much more often than s/he uses the same app on
different OS's. So when there's a conflict, it is more important to fit in  
with the conventions of the OS than with the way the app
works on another OS. Lack of attention to this has been a serious complaint  
about Mac ports in the past (MS Word 6 is the
standard horror story.)
