This is done via the console serial port, when you don't have a network
interface for TFTP.  

After you give the xmodem command on the router, it will be ready to 
receive the file over its console port.  Then you need to use your 
communications program (minicom, CRT, teraterm, hyperterm, etc.) to
start sending the file.  The exact syntax depends on the terminal
program you're using.  Check your communications program documentation 
on how to upload a file using xmodem.  

Uploading images this way is a slow and painful process, btw.
