Is the port adapter a NM-2W= ? This holds 2 WAN slots... Check out

According to this, you have to have IOS release 12.0(7)XK or higher for the
2-WAN card slots. I'm willing to bet the IOS release is older, without even
knowing anything else about the problem. Was I right?
