Hey Kevin,
As for the US, you can go to http://www.arin.net/whois/ and type in the IP
address. That will in most cases give you the ISP that the user/company
you're looking for leases their IP block from, and you should be able to
contact their administrative contact and request information about the IP
address/block you're interested in - especially if you've had a hacker
attach, etc.
As for outside US, you can try http://www.ripe.net/cgi-bin/whois or
http://www.apnic.net/.
