Vic, One of your 4Mb Flash Modules might be bad, did you try using them alone? I had the same problem on a 2600, but with 2x8Mb Flash config. What I did was: using a third 8Mb Flash Module, replaced one module at a time, reload the IOS, reload the router until it worked.
If you can't try that, try using your 4 Mb Flash modules with an IOS that would fit in 4 Mb and test them separately.
