The flat chests are not the cause, they are the result of being a swimmer.
If you notice a puppy not lying on its side as it should and treat it
immediately it will not develop the flat chest. Also, swimmers can occur in
litters of 4 or less. I don't think that litter size has anything to do with
it. However, slick footing can definitely cause swimmers. It is not the
cause in every case though.
