/var/log/maillog. If there's nothing in the log it probably isn't running.

I found the clamav-milter doesnt log anything until the "LogSyslog" is set on in clamav.conf.
It is a feature, I guess. But another logfile will be better, maybe.
Can you change clamav-milter behaviour to take into account the clamav.conf setting, please?

I think the good logging is very important for bug hunting and will increase the quality of bug reports.
