We saw that message when database corruption occurred during an upgrade
of our CAM. According to TAC in our case the corruption apparently
occurred during the upgrade to 4.1.2.x but didn't show itself until we
did the following upgrade to 4.1.3.x. In our case we had to roll back to
4.1.2.x and spend time finding the last usable backup.
I believe the "showstate.sh" script contained in the upgrade tarballs
can show you if the appropriate database changes were made after the
upgrade is completed.
