In your case did the showstate.sh report problems with the database or schema? I ran the script on our CAM and the only items that did not come back as CORRECT were related to the updated agents:

According to TAC in our case the corruption apparently occurred during the upgrade to 4.1.2.x but didn't show itself until we did the following upgrade to 4.1.3.x. In our case we had to roll back to 4.1.2.x and spend time finding the last usable backup. I believe the "showstate.sh" script contained in the upgrade tarballs can show you if the appropriate database changes were made after the upgrade is completed. -- Bruce A.
