Are you doing OOB? If so, are you blocking 8905/udp and 8906/udp on the access VLAN ACL? If you don't do that it will cause symptoms similar to what you're describing.

We did make the jump to 4.1.6 , but I don’t think the code is inherently broken, I think it is my setup… I have deleted/recreated the login page thinking it may be just strange, but I don’t know where else to go… it almost seems as if my CAS isn’t even attempting authentication.
