Unfortunately,
I am running In-Band... Not sure what the heck is going on... kinda waiting for 
the 4.1.6 release to be visible in the software download table without 
"guessing" where it is so I can call the problem in.
I think if i dont have a solution by tomorrow, I will probably be reverting to 
4.1.3 where I can restore from my backup and things "should" work.

Bradley,

Are you doing OOB? If so, are you blocking 8905/udp and 8906/udp on the
access VLAN ACL? If you don't do that it will cause symptoms similar to
what you're describing.
