The manual update only worked once for some reason.

Solution:
a: run the agent as admin, don't know why I didn't think of that.
or
b: grant the user permissions to
c:\programdata\microsoft\microsoftforefront\ etc down to the definition
directory.  I noticed that as I browsed there and the Windows "you must
grant yourself admin privledges on this folder" dialog popped up, it
immediately started working.
