Updating this purely for the record, and as a reminder that the problems aren't always where we are looking.

OK, nvidia and apple are innocent. My mouse was worn out! Funnily enough, my other (PS/2) boxes didn't have any problems (apart from my difficulty in moving the mouse cursor to the left), but it looks as if somewhere in the USB subsystem it was getting overloaded. The old mouse had three internal rollers, one of them seemed to have become hard to rotate in one direction (like a ratchet). I've now found my spare mouse, and it's been working perfectly for about three hours.
