I've had qmail-scanner running for the past couple of weeks, with clamav and 
SpamAssassin.  I have my local.cf file setup for SpamAssassin as well.

When I check the headers of the incoming email - spam and non-spam, I'm seeing 
somthing similar to the following:

+qmail-scanner-1.16
 (clamscan: 0.60. spamassassin: 2.60.  Clear:SA:0(0/0):.
 Processed in 5.98307 secs); 01 Mar 2004 12:51:29 -0000
X-Spam-Status: No, hits=0 required=0

The weird part is the scores for clamscan and spamasassin NEVER change.  Which means 
that spam is not getting tagged (which in other situations would be a matter of 
training or tweaking the local.cf file).  I would expect the SpamAssassin score to 
change for each and every email.  Has anyone else seen this?  I suspect the way 
SpamAssassin is getting called, my local.cf file isn't being implemented, but don't 
know where to look for this.

I do have the recommended entries in the tcp.smtp file (i.e. 
192.168.0.:allow,RELAYCLIENT="",RBLSMTPD="",QMAILQUEUE="/var/qmail/bin/qmail-scanner-queue.pl"),
 but am not aware of any qmail-scanner specific config files.
