If she was out of the facility for an LOA overnight, you will just continue her Medicare A. You are correct that you cannot bill for the LOA day.(the midnight she was out). Be sure to adjust your Medicare A days (you will not count the LOA day, so she will end her 100th day (if she remains skilled) one day later. Also look at your upcoming assessments as you may have to move your ARD to have 5 days of therapy in your observation period.
