I personally would track down the info and use it.  I would clarify the
situation in a progress note.  The very nature of tx with a UNNA boot
prohibits us from removing it to just "have a peek" yet we have to code
on the MDS which is a care planning tool and we know the ulcers are
there. Also if it is a PPS assessment you need to support the skilled
care aspect to get the RUGS etc.  If I were unable to obtain the
information I am not sure but UNNA boots are not applied to intact skin
so it has to be at least a stage 2 or would we go the unstageable "4"
route. Rena?  We are also lucky in that the MD who sees our residents
with UNNA boots is very responsive to our documentation needs. WE get
exact measurements and staging info.

I called the vascular Dr's office and asked that each time the patient

for a change of the boots that the Dr write and draw the extent of the
area
on the referral that we send.I guess I was lucky because it happened.I
used
the Dr's description.Perhaps the nurse did it but Dr signed.
