### What problem does this PR solve?
Issue Number: None
Problem Summary: An explicit `REFRESH TABLE` invalidated connector metadata
but left the external database's cached table-name snapshot and table object/ID
lookup in place. If a remote table was dropped or recreated under the same
name, `SHOW TABLES` and subsequent lookups could continue to use stale FE
identity until cache expiry. This change invalidates the lookup state on
explicit refresh and replays the same decision on follower FEs. It also covers
cold replay and case-insensitive names, where an old spelling can refer to a
different cached identity. Internal refreshes after content mutations keep
their existing, narrower invalidation; edit logs without the new flag retain
that behavior.
### Release note
Explicit `REFRESH TABLE` now reloads external table names and table lookup
identity.
