Fixed in eae234d: getColumnNames() now uses the constructed data-source
keys, which include the logical OPEN_STRUCT parent and exclude materialized
children. Added an existing-suite regression test.

Fixed in eae234d: mapped column views are now lazily cached per ordinal with
safe publication. The mapped round-trip test checks repeated lookups return the
same view.

Fixed in eae234d: index sizes are decoded once per mapped column view, then
indexed directly.

Review Comment:
Fixed in eae234d: the contribution cap now counts and filters physical
columns, matching the stats reader. Added a focused assertion in the existing
test.
