### Problem / Evidence

### Root cause / Fix
The read result is stored without the cluster it belongs to, and the error
path was toast-only. Clear the panel when an examine fails and show
`clusterConfig.clusterId` in the summary row, so the panel always says which
cluster it describes.
### Priority and scoring
**PRIORITY 45** - impact 16/40 (the operator reads another cluster's
accounts and verdict), blast radius 6/20 (one panel), reproducibility 18/20
(pinned by the new test), maintenance value 5/20. **FIX_CONFIDENCE 72**.
### Tests
`cd web && npx vitest run src/pages/instance/__tests__/AclPage.test.tsx` ->
`Tests 27 passed`; the new case fails before the change and passes after it;
`npx eslint` and `npx tsc -b` are clean.
### Risk
A failed examine now empties the panel instead of showing stale data; the
success path is unchanged.
