Ok guys I figured out the problem. I'm using jQuery to submit my forms, and I was accidentally submitting two "MainForm" forms at once, upon log-in.
HOWEVER! The real problem lies in why HopToad is not reporting this exception to me? It is indeed in Heroku logs.
