Before System 7, the Apple Menu was a big hack and was reserved for use by Apple's own bits (eg Control Panel), Desk Accessories and the application About box. With System 7 it's less of a hack but still messy. It would be possible to write a DA or a separate app that acted as a program launcher under Multifinder so look through the old UMich archives -- there was an app that resembled the NeXt boxes program launcher, for example.
