What System version were you using? It has happened the same to me using a clean System 7 boot disk, so I think Sim020 is compatible only with System 6. It could be also a conflict with other INIT/CDEV... If you're using System 6 try to disable all other extensions and enable it one after other - you'll find the guilty one that way.
