A couple thoughts on this:

1) It might be good to run concordance under gdb:

gdb concordance <<args>>

Once it segfaults, you can type "bt" to get some more information about
the crash, and the "q" to quit.

2) After the lsusb command Phil mentioned above, try unplugging the
Harmony from the USB connection, waiting a few seconds, and then
plugging it back in. Then run the same lsusb command again.
