add contributors to CHANGES.txt

As mentioned on the connectors-dev@ list (change the format of CHANGES.txt), I
propose we modify CHANGES.txt
to give credit to contributors who have given bug reports, comments, patches,
etc.
I'll volunteer to go thru the mail archives and jira issues that are marked
'resolved' and upload a patch here.
