Ok, I ran modprobe loop and then
 mkinitrd initrd-2.4.19-13mdk.img 2.4.19-13mdk  and
lilo

When I rebooted I got the following errors, with the bootsplash I saw
Kernel panic VFS: Unable to mount root fs on 03:06
in failsafe and linux-nonfb I got
Kernel panic: No init found. Try passing init= option to kernel
( I really have no idea what this is telling me to do)

I was able to boot with 2.4.19-3mdk, same kernel I have been running for a 
long time.

I was both reporting an issue and asking for a workaround.  I still have the 
problem if I boot into either 12mdk or 13mdk.  If I can help diagnose an 
issue I am more than happy to help.
