Attached is aircraft specification A-787 which spells out (officially)
just what you need to do to convert from a 415-C to a 415-D. You'll need
Adobe Acrobat Reader (a free public domain program http://www.adobe.com)
to read it.

Your plane may be old enough to not have the aerodynamic trim tab.  You
must have the aerodynamic trim tab, the skin over the header tank
replaced with stainless steel, the up-elevator travel limited to 9
degrees and other minor changes.

The change from C to D is specified in the type certificate and should
be straight forward.

I think the conversion to the E model's split elevator is an absolutely
good idea with no drawbacks (though not necessary) but I've heard of an
FAA paper-pusher rejecting that conversion -- staff the paperwork on
that through the FAA --before-- you make the split elevator conversion.
(I've wanted to do that conversion for 20 years but it hasn't come near
the top of my priority list.)

Skyport is a good place. I try to do all my business with them, only
going to other vendors if Skyport doesn't have what I want. Call quick
and get the catalog.
