Because it's much simpler to see what it looks like without having to
build your local cpan.io, I've put the code live with only Neil's history
of CPAN, which cpan.io was already publishing.

The benefit is that now we're back to having a single authoritative
source for that document. I've sent Neil a pull request with the patches
I received for my version of the document.

The only remaining commit on the book/publishing branch is the one that
enables publishing the content of the toolchain-site repository. I'll
only apply that if David agrees.
