Funding Year 2002 (July 1, 2002 - June 30, 2003) At this time, most applicants for Funding Year 2002 have already received a Funding Commitment Decision Letter (FCDL). This letter tells you whether your funding requests have been approved, denied, or partially funded. If you have already received your FCDL for services beginning July 1, 2002, it is time to file the Form 486. The Form 486 is the third form in the E-Rate filing process. A Form 486 must be filed within 120 days of service start date or the date of the FCDL, whichever is later. For example, if you: Received FCDL on May 15, 2002 and the service start date is July 1, 2002, then the 486 deadline is October 28, 2002. (120 days after July 1st service start date) For example, if you: Received FCDL on August 15, 2002 and the service start date is July 1, 2002, then the 486 deadline is December 13, 2002. (120 days after the date of the FCDL). For example, if you: Received your FCDL on July 1, 2002 and the service start date is September 1, 2002, then the 486 deadline is December 29, 2002 (120 days after the service start date of September 1st) If you have not already received an FCDL, then you will have 120 from the date of that letter to file the Form 486. If your services started July 1, 2002 and you received your FCDL prior to that date, you must file your Form 486 by October 28, 2002. There are two versions of the Form 486 that the SLD will accept for Funding Year 2002. Each form has a date in the lower right hand corner. For Funding Year 2002, the SLD will accept Form 486 versions dated July 2001 or September 2002. For future Funding Years, the July 2001 form will not be accepted. Recommendations: File your Form 486 as soon as possible after receiving your FCDL.
