Following the mixup last time this time I've tested my DBD::Proxy
patches a bit more.

However, I have only tested against an underlying DBD::ODBC talking to
M$SQL.

I would apprciate if someone out there could try these aginst another
couple of DBDs before Tim merges them into the source tree.

With this patch DBD::Proxy server allows for the possibility that the
underlying DBD may not know NUM_OF_FIELDS and NUM_OF_PARAMS until
after execute().

This patch also allows for the fact that some underlying DBDs can
accept a wider variety of statements via do() than they can via
prepare()/execute(). In the case of DBD::ODBC and M$SQL this applies
to "CREATE VIEW".

I still haven't got arround to making DBD::Proxy proxy the
execute_array() in a sensible way.
