>From your log file, you have an old Digital TGA2 graphics card, officially known as either a ZLXp-E1 or ZLXp-E3. I can't tell which flavor from your log. The E1 was an 8 plane board with 2MB of memory. The E3 was a 24 bit with Z buffer and could do 12 bit double buffering. It has 16MB of memory.
DEC released drivers for them and at one point, and it was supported in XFree86 in later releases. I don't know if anyone has ever ported them to the XOrg server though. Feel free to take a whack at it.
