After the recent security update, squid3 has been failing requests for some
items, apparently crashing & restarting. One time it failed to restart at all.
I am unable to identify a common footprint so far, and re-requesting the same
item seems to work. Examining the cache log shows repeated occurrences of this
message sequence:
