In addition to the wiki page you found already there is also a chapter in the wonderful new book on Debian system administration on kernel compilation [0] and a slightly older chapter in the kernel handbook [1]. The latter also discusses the application of patches [2]. It is typically also a good idea to read the README.source in the source package. I am not entirely sure which procedure to recommend as I had the impression that make-kpkg wasn't the recommended way to build a kernel these days.
