Actually Borland does install it by default (at least in D6), they just don't tell you about, put any links to it, or supply any info about it at all! I think I discovered by accident.
Look under Help|Tools in your delphi directory. HCW.EXE is the Microsoft Help Compiler, SHED.EXE is the hotspot compiler (for providing a graphic of your app with hotspots that pop up help windows), and there's a help dialog box editor which works with Word. From what I remember it was a matter of adding specially formatted footnotes on each page of your word doc, double underlining words in green text to make clickable links, and it was quite easy to sync up with the help numbers that are a property of each control in a Delphi app. once I had it all figured out.
