After re-reading Kathey's comments in DERBY-3105, I'm tempted to leave the
-ca implementation present, and just remove it from the usage message. That way
the option will still work, if somebody is somehow depending on it from back in
the Cloudscape days, but it won't confuse new users who encounter the usage
message.
