I think about this problem: 1. "96_pixels" pictures are not created. Windows-OS do not use this size. Windows Vista and 7 make less "256px.png" view of the icon to the size of 96 pixels. Some of the icons ("256px.png") are then blurred. 2. Create a new "256px.svg" is not necessary. "256px.png" can be drawn from "128px.svg".
