I couldn't open that link in Evince 3.4.0, but I get the same problem with this 
PDF (and all the other PDF:s with forms that I tried so far):

Just wanted to add that it seems like the but persists in 3.4.0 as well.
I have a couple of variants of this PDF saved since I edited them in
Evince for Ubuntu 10.10, and when I save them, they are saved exactly as
if I didn't change them, that is what the file looked like when I opened
it.
